New Students

Welcome

Students on any Mercer campus in need of any accommodation must follow the office procedures.

  1. Disclose your disability by completing our application requesting accommodations.
  2. Schedule an interview with the Director/Coordinator for you campus/school to discuss your experiences.
  3. Submit documentation from a qualified provider. To assist in gathering documentation you may download our disability verification form and have your provider complete.     Documentation requirements for different disabilities
    Disability Category Appropriate Provider Documentation requirements
    ADD/ADHD Licensed psychologist or treating healthcare provider Comprehensive Psychological Evaluation dated within 3 years OR the Disability Verification Form (DVF), dated within the previous year
    Allergies or Asthma Allergist or pulmonologist Allergy results dated within three years
    Chronic Health Conditions Treating healthcare provider (licensed medical specialist: MD, DO, NP, PA, etc.) DVF, dated within the previous year.
    Hearing Impairment or Deafness Audiologist Audiogram, dated within the previous year
    Learning Disabilities Licensed psychologist Comprehensive Psychological Evaluation Report with subtest scores, dated within three years
    Psychological Disabilities (other than ADD) Counselor/therapist, psychologist, or psychiatrist (licensed mental healthcare professional) DVF, dated within the previous year.
    Visual Impairment or Blindness Ophthalmologist Recent acuity and field of vision, dated within three years
  4. Approval Meeting Needed – if verified following the review of documentation
  5. Submit a semester request to notify instructors or staff of the accommodations approved. Following the delivery of the accommodation letter students must meet with faculty to determine the implementation of accommodations.

Please remember that accommodations are not provided until approved and are not retroactive in nature.
